Community Health Foundation of Greater Petaluma is committed to providing grant monies in support of local resources that promote community health in South County (Petaluma and surrounding rural areas.) Our priorities are: (1) programs and services that support youth to be actively engaged in the community, make healthy lifestyle choices and prevent at-risk behaviors; (2) programs that enable seniors to live independently and have available a range of supportive services; and (3) community services that are accessible to any community member who needs help in managing the daily challenges of living a healthful life.
CHF will announce the amount of grant monies that are available by February 1, 2010 that will be distributed to grantees by July 15th of the same year. In 2010, a total of $125,500 will be awarded in grants. Eligible current grantees will receive priority in the grant process. Organizations that received funds in the prior grant cycle have been asked to submit progress reports due January 15th, which combined with a site visit, will determine future funding. Community grants are made through a competitive process. A decision will be made on February 17, 2010, whether to invite organizations that are not current grantees to submit Letters of Inquiry.
CHF has decided that our grant program will prioritize our recent grantees to provide stability during these challenging times. We will not be inviting letters of inquiry for this grant cycle. Please check back in September, 2010 for information about the next grant cycle, which will begin with a Letter of Inquiry. The LOI should be no longer than two pages and provide the following information:
The Community Grants Committee will review all the above information and contact organizations about what additional information may be needed, including full proposals from organizations not currently grantees. Site visits will also be conducted for each organization invited to submit a full proposal.
Decisions will be made by April 15th and monies distributed the following July. Narrative reports are required to document use and impact of monies received.
Applicants may contact Suzanne Cochrane, Project Coordinator, at 707.773.3863 with questions about the Letter of Inquiry or application process.
